Survey on "Computer Use in Your Organization"

I've modified a survey that Elaine Dockens (library director for Tressler LLP in Chicago) did for law librarians last month. I adjusted to make the survey more "generic" and wanted to survey the SLA-Texas Chapter to see what the trends are within our diverse libraries.

Please take a few minutes to complete the survey (most are simple "Yes/No" questions). If you can, please make any comments you feel are important to explain your answers.

Here are the Questions you'll find on the survey:
Does your organization attempt to control computer use?
Are there different computer use rules for different groups of people? (For example, different rules for professionals, different rules for librarians and research staff, different rules for secretaries, etc.)
Do the rules/guidelines block access to certain websites in general ?, e.g., pornography, gambling, etc.?
Do the rules/guidelines block access to certain websites by specific URL?
Does your organization block access to personal email accounts like Gmail, AOL, etc.?
Does anyone in your organization report using personal e-mail accounts for business purposes?
Does your organization block access to social networking sites?
Does anyone in your organization report using or requiring social networking sites for business purposes?
Does your organization block access to streaming media sites like You Tube?
Does anyone in your organization report using or requiring streaming media for business related Current Awareness?
Does your organization have an “Acceptable Use Policy” regarding computer use, email and internet?
What exceptions, if any, are allowed regarding the “Acceptable Use Policy”?
What issues or problems were encountered with blocking sites or controlling access?
If there was resistance from groups of people (professionals, librarians, secretaries) how was such resistance handled or avoided?
Approximate number of professionals in your organization?
Who makes the decisions regarding computer use(personal and/or business) e.g. one individual (e.g. CEO, managing partner), an “Executive Committee” with total company control; a Committee of Executives and/or Managers with responsibility over computer use only, or another decision maker?
Can you recommend a list serv used primarily by IT directors, network administrators, etc.
